How do I access these supports?
Each pathway is designed for a specific situation and must be linked to your assessed needs. To access short-term support, you'll need a My Aged Care assessment, which helps determine the services and funding that may be right for you.
Depending on your circumstances, you may be recommended for Restorative Care, Assistive Technology and Home Modifications, or the End-of-Life Pathway. If you're unsure which supports may be available to you, VMCH can help you understand your options and navigate the assessment process.
